The answer depends on your vial size and target concentration. Here are the standard reconstitution volumes, dose tables, and draw unit calculations for every common tirzepatide vial size.
For a 5mg vial (most common):
General rule:
Resulting concentration in mcg/mL based on vial size and BAC water volume added.
| Vial size | 1mL BAC water | 2mL BAC water | Notes |
|---|---|---|---|
| 2.5mg | 2,500 mcg/mL | 1,250 mcg/mL | Starter dose vial |
| 5mg | 5,000 mcg/mL | 2,500 mcg/mL | Standard vial |
| 7.5mg | 7,500 mcg/mL | 3,750 mcg/mL | |
| 10mg | 10,000 mcg/mL | 5,000 mcg/mL | |
| 12.5mg | 12,500 mcg/mL | 6,250 mcg/mL | |
| 15mg | 15,000 mcg/mL | 7,500 mcg/mL | Max approved dose |

Let the vial reach room temperature
Remove from refrigeration 15–30 minutes before mixing. Cold peptide and room-temperature BAC water can affect stability.
Draw the correct volume of BAC water
Using a fresh needle and syringe, draw 1mL (or 2mL, based on your target concentration) of bacteriostatic water.
Inject BAC water slowly into the vial
Angle the needle so the water runs down the side of the vial — do not inject directly onto the lyophilized powder. Add slowly to avoid foaming.
Do not shake — gently swirl
Shaking can degrade peptides. Roll the vial gently between your palms or swirl until the powder is fully dissolved. The solution should be clear and colorless.
Log the reconstitution date immediately
The 28-day clock starts now. Log the date, BAC water volume, vial size, and resulting concentration before refrigerating. Protocol handles this in the vial management screen.
Refrigerate at 2–8°C (36–46°F)
Do not freeze. Use within 28 days. Protect from light.
