How to Calculate Compounded Semaglutide Doses

Compounded semaglutide comes in different concentrations. Your vial label shows the concentration in mg/mL. Use this to calculate draw units for any dose.

Draw volume (mL) = Dose (mg) ÷ Concentration (mg/mL)
Draw units = Draw volume (mL) × 100 (U-100 syringe)

Example: 2.5mg/mL vial, 0.25mg starting dose

Draw volume = 0.25mg ÷ 2.5mg/mL = 0.1mL

Draw units = 0.1mL × 100 = 10 units

How do I calculate my semaglutide dose?+
For compounded semaglutide: (1) Find your vial concentration in mg/mL on the pharmacy label. (2) Divide target dose mg by concentration to get draw volume mL. (3) Multiply by 100 for U-100 syringe units. Example: 2.5mg/mL vial, 0.25mg dose: 0.25 ÷ 2.5 = 0.1mL = 10 units on U-100.
What is the standard semaglutide titration schedule?+
Wegovy (weight management) titration: 0.25mg weekly for 4 weeks, then 0.5mg → 1mg → 1.7mg → 2.4mg maintenance, each step lasting 4 weeks. Ozempic (diabetes) starts at 0.25mg weekly for 4 weeks, then 0.5mg. Compounded semaglutide titrations vary by prescriber.
How do I reconstitute compounded semaglutide?+
Compounded semaglutide is typically supplied as lyophilized powder or pre-mixed solution. Follow your compounding pharmacy's specific instructions. If reconstituting: add specified BAC water volume slowly along the vial wall, gently swirl (do not shake), refrigerate immediately. Use within 28 days.
